Optimal diagnosis and management of paronychia

According to literature reports, paronychia is one the most frequent hand infection, representing 35% of these disorders and more common in women. The pathology is described as inflammation or infection of the proximal/lateral nail folds. Any trauma such as nail biting, aggressive manicuring and hangnail manipulation are common causes of acute paronychia with Staphylococcus aureus being the most common pathogen. If the process of inflammation takes more than 6 weeks, it is considered a chronic type. This may result in nail ridging, discolouration and rounding, whereas cuticle may separate from the nail plate. Conservative approach is often sufficient for acute paronychia without abscess- warm soaks in water, vinegar or antiseptic solutions. Cases with abscess require incision and drainage. Chronic paronychia may be managed with topical steroids.
