Firstly, the chemical composition of products vary greatly and is not regulated. The breakdown of heated propylene glycol include formaldehyde that is a known carcinogen, causing 5 to 15 times higher lifetime risk of cancer. As for the flavorings, some research is demonstrating those have a cytotoxic effect. Secondly, there are device-specific concerns, as unregulated manufacturing process and basic design. The long term effects of metals used for a heating element are still unknown. As such, the perception that vaping is safe is changing.
